What is a Juror Qualification Questionnaire and why did I receive it?
The Juror Qualification Questionnaire is not a summons for jury duty. The Juror Qualification Questionnaire is one of the first steps in the juror selection process. Potential jurors are selected randomly from the State of Michigan Driver’s License list and Personal Identification Cardholder list for individuals residing in Emmet County. Once the questionnaire is completed, returned, and evaluated by our Jury Board, a citizen may be called to serve on a jury.
Do I have to return the Juror Qualification Questionnaire?
You are required by law to return the completed Juror Qualification Questionnaire. Failure to return the questionnaire could result in a Show Cause Order. Failure to respond to the Show Cause Order may lead to a bench warrant for your arrest.
How soon must I return the Juror Qualification Questionnaire?
Jurors must return the questionnaire within 10 days of receipt. A self-addressed mailing envelope is provided with the questionnaire.
What if I am no longer an Emmet County resident?
If you are no longer living in Emmet County, please write “No Longer an Emmet County resident” on the questionnaire, and attach a copy of your updated Driver’s License showing your change of address (front and back).
The Juror Qualification Questionnaire is addressed to my son/daughter who is away at college. What should I do?
Forward the questionnaire to your son/daughter at college and have him/her return the completed questionnaire to us. The student will need to indicate that he/she is a college student on “Occupation” line #3 on the back of the questionnaire.

What if I have a medical or physical condition that would prevent me from serving as a juror? Complete question number 4 (four) on the questionnaire. Any physical or mental disability requires a written statement from a physician declaring the nature and duration of your condition.
I am over the age of 70. Do I have to serve on jury duty?
A person over the age of 70 may claim an exemption from jury service and will be excused upon checking the NO box to question number 5 (five) on the questionnaire. If you want to remain in the jury pool for Emmet County, mark the YES box.
What if the person who is sent the Juror Qualification Questionnaire is in the Military?
If a member of the ARMED FORCES is sent a questionnaire, write that information on the questionnaire, sign your name, and return in the self-addressed envelope.
The Juror Qualification Questionnaire was sent to a family member who is deceased, what should I do? Write “deceased” on the questionnaire and return it in the envelope provided.
